Foundations of social, cultural, behavioral, genomic, political, and environmental factors impacting health. Exploration of provider roles in assessing the health of individuals and aggregates in planning health promotion, disease prevention, and health maintenance programs with a focus on chronic conditions and vulnerable, rural, and underserved populations.
